Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez arrived at facility to conduct an unannounced annual inspection visit. LPA called the Administrator (ADM) Flamingo Ligutom who stated she is not available and will send Designated Administrator to meet at the facility. LPA met with Eddilyn Realista, DA

ADM stated the facility does not currently have any residents. LPA stated the department needs to conduct the annual inspection even if there are no residents.

During visit, LPA toured the facility inside and out. LPA toured the garage area and observed food storage areas and locked cabinets for cleaning supplies. LPA observed the kitchen area and observed locked cabinets for medications, sharp objects, and cleaning supplies

LPA toured 4 resident bedrooms. Each bedroom had available bedding and clothing storage areas as well as functioning lights. DA tested the smoke detector and found the smoke detector to function properly when tested. LPA toured two out of two resident bathrooms. Each bathroom had available soap and paper towels and functioning lights. The water temperature in the bathroom sink measured with thermometer at 125 degrees F. LPA reminded DA that water temperature needs to be between 105-120 Degrees F. DA turned the water heater down and water regulated between 105 and 120 degrees during visit.

DA stated the facility is vendorized by San Andreas Regional Center but HCBS is stating they want a new fire clearance stating room 3 has been updated to include a window in place of the current exit door.

No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with DA and a copy of this report was provided.
