Facility Evaluation Report

Licensing Program Analyst Manuel Monter conducted an unannounced case management- other regarding the facility’s hospice waiver request. LPA met with Administrator Wennie Concepcion and explained the purpose of the meeting.

During todays visit, LPA discussed with ADM the status of the hospice waiver. LPA discussed with the licensee hospice regulations.

LPA informed ADM the facility hospice plan that was provided to the department is insufficient: due to having incorrect codes and cites older code sections that are not applicable.

LPA informed ADM she has the option to reapply in 6 months.

No deficiency was cited during todays visit. This report was reviewed with Administrator Wennie Concepcion. A copy of this report was provided.
