Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management visit for an incident report. LPA announced the purpose of the visit and met with Licensee Margie Villanueva. LPA observed 2 staff, 6 residents in living room and 2 residents in the dining area.
On 09/30/25 the department received an incident report that occurred on 09/23/25 for Resident (R1) who left the facility at 2pm and did not return for bedtime medication. On 09/24/25 Licensee called local law enforcement for missing persons. Local law enforcement was able to communicate with R1s son, who was able to verify R1 was in his backyard the night before and at his home. Licensee stated that R1 gets depressed and will go and visit son. Per residents physicians report resident is allowed to leave unattended.
Licensee stated that R1 has not had an episode of leaving the facility and not returning. Licensee stated in the future R1 will inform staff when and where he/she is going when he/she leaves the facility.
LPA determined that the above incident does not require further investigation because resident is allowed to leave facility unattended. No deficiencies were cited at this time as per California Code of Regulations Title 22.
