The inspector’s account

Licensing Program Analyst (LPA) Maria (Mita) Partoza, conducted an unannounced required-1 year inspection visit, met with administrator (ADM) Recto Sandoval & Rubenita Sandoval and LPA stated the purpose of the visit.

The facility is licensed to serve adults 18 to 59 years old wherein all may be non-ambulatory.

At 9:40 a.m. LPA toured the facility accompanied by ADM, inside and outside. LPA observed the exterior is free from debris, walkways are free from obstruction. LPA observed a newly built Added Dwelling Unit (ADU) that is permitted by the city of San Jose and will be used as a staff in-dwelling unit. ADM stated that the ADU will not be used by residents in care.

LPA toured the interior of the facility including but not limited to the kitchen, dining, 2 1/2 bathroom, 5 resident bedroom, living and dining room, garage/staff room break room, and laundry room. LPA observed sharps, chemicals and laundry supplies were stored and locked not accessible to any of the resident.

Residents bedroom were organized and sanitary, with storage to for personal items. 1 Out of 5 resident room is a shared room and 4 out of 5 are single occupancy. A locked room is observed by LPA and observed that the room is used as a storage of facility supply overflow.

Room facility room temperature is at 74 degree F, water temperature measured at 120 degree F. Refrigerator temperature measured at 32 degree F and freezer temperature measured at 0 degree F. The kitchen is observed to be organized and sanitary, dry food pantry was observed to have 7 days of non-perishable food items and refrigerator/freezer has 2 days of perishable items. LPA observed 2 freezers in the exterior of the facility for additional food items.
