Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced case management visit and met with House Manager Recto Sandoval (HM).

LPA observed 2 staff in the facility and all residents went to day program.

On 5/6/2025, the Department received a notice from Administrator (ADM) Ruby Sandoval with related document that the facility will install a prefabricated one bedroom and one bathroom ADU at the back yard. . On 5/7/2025. ADM sent some update document for the project.

LPA interviewed HM. HM stated the project already obtains the city permit. HM stated on 5/6/2025, fences were put at backyard to secure the premise. HM stated 5/8/2025 and 5/9/2025, layout was put in the backyard.

LPA toured the facility and backyard with HM, LPA observed the backyard was secured by fences and the facility has no access to the backyard. HM stated there is no construction work at the backyard because it is a prefabricated ADU unit. HM stated it only needs installation work of the prefabricated ADU unit.

LPA requested an action plan to reduce the impact and to provide the safety and health to facility residents.

Exit interview was conducted with HM. A copy of the report was provided to HM for review and signature. A copy of the report was provided to HM.
