On 07/14/25 Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced POC visit. LPA announced the purpose of the visit. LPA met with Administrator Ying Wang. LPA observed 3 residents and 2 staff.
on 06/19/25 the facility was cited for type B deficiency for not obtaining an exception for a resident having Full Bed Rails
On 07/14/25, LPA conducted a POC visit to clear the Type B deficiency. A copy of the Letter of Deficiencies Cleared letter was provided.
No deficiencies cited during today's visit. This report was reviewed with Administrator, Ying Wang copy of the signed report was provided.
