Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced required 1 year visit. LPA arrived at the facility at 3:23 PM. LPA called the Administrator Cyril Inneh and asked to meet at the facility for LPA to conduct the annual inspection.

During visit LPA observed the facility front windows and side windows are boarded up and does not have any residents currently living in the facility. The facility is undergoing renovations. The facility is non operational. ADM stated there are no residents currently living in the facility since April 2022. ADM stated the facility needed required renovations.

No deficiencies were cited at this time as per California Code of Regulations Title 22.
