Facility Evaluation Report
On 10/20/2025, LPA Maria (Mita) Partoza, conducted an unannounced POC inspection visit and met with licensee/administrator Armando Silva. LPA stated the purpose of the visit.
LPA toured the facility with LIC/ADM inside and outside to address the deficiencies cited during the annual inspection. Based on today's inspection,observation and record review the facility corrected and cleared the deficiencies that were cited. LPA issued the Letter of Deficiency Citations Cleared during today's visit.
Based on observation - the back ramp that was missing planks were repaired and planks that poses a potential risk of breaking were replaced. Based on observation the boxes of perishable food at the side of the ramp were removed and cleared. The water temperature is monitored on a daily basis to ensure that temperature stays within the range of 105 to 120 degree F. LPA measured the water temperature in the bathroom and kitchen with a digital thermometer. Temperature measured at 110.1 degree F.
Based on record review - 3 out of 3 staff CPR 1st aid training have been completed and 3 out of 3 staff have a valid 1st aid/cpr certificate that will expire on 10/16//2027. 3 Out of 3 staff was provided the required 10 hours of continuing education. ADM started the training for 3 out of 3 staff and administered on 10/20/2025 and completed on 10/27/2025. 3 out of 3 staff training for the dementia care is ongoing and will be completed on 10/31/2025. LIC/ADM stated that he/she will send certificate of completion to LPA by 10/31/2025.
No deficiencies were cited during today's visit based on California Code of Regulation (CCR) Title 22. This report was reviewed by LIC/ADM Armando Silva and a copy was provided.
