Licensing Program Analyst Manuel Monter and Simi Rai conducted an unannounced case management to amend a Complaint investigation, LIC9099, LIC9099-C and LIC9099-D issued on July 15, 2025. LPA met with Administrator Eliza Daquioag explained the purpose of the visit.
The complaint investigation closed on July 15, 2025 is being amended and re-opened due to new information provided to the Department.
No deficiencies cited during todays visit. This Report was reviewed with Administrator Eliza Daquioag. A signed copy was provided.
