Facility Evaluation Report
On July 29, 2026, Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced case management visit -other regarding the facility’s request for a change in capacity. LPA met with Administrator Cecilia Valenzuela . LPA explained the purpose of the visit.
On June 16, 2026, LPA Manuel Monter received an email from Licensee Jalano, which requested the steps and required documents to change his facility capacity from 4 to 6. On June 17, 2026, LPA Manuel Monter informed Licensee Jalano what was required to update his facility’s capacity from 4 to 6.
On July 22, 2026, LPA Manuel Monter received some of the requested documents from the Licensee such as but not limited to: request letter, facility sketch, LIC200.
On July 28, 2026, Licensing Program Manager Romeo Manzano and LPA Manuel Monter contacted ADM Cecilia Valenzuela. ADM stated that an additional wall had been added to the office in order to create a new staff rooms.
On July 29, 2026, Licensing Program Analyst Manuel Monter toured the facility inside and out and met with Licensee Jalano Valenzuela, referred to as LN. LN stated the added walls in the office have a permit. LN stated the office will now be two staff rooms. LN stated the former 2 staff rooms in bedroom 5 & 6 will be resident bedrooms.
