Facility Evaluation Report
Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced annual inspection visit, and met with Administrator (ADM) Izumi Yaskawa. During the visit, LPA observed 3 residents and 4 staff. LPA explained the purpose of the visit.
LPA toured the facility inside out with ADM which the Program room, kitchen, office, outdoor deck and restrooms . There was no obstruction to block the walkways. The staff area of the facility was also inspected. The front yard and backyard was inspected.
LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 75 degrees F, and hot water temperature was measured at 110 degrees F in both resident bathrooms. ADM stated the facility does provide meals to residents during their stay at the day program.
Fire extinguisher was serviced in June 12, 2026. The facility's sprinkler system was last inspected on February 12, 2026. LPA observed facility first aid kit and facility fire/earthquake drill log. The facility's last drill was on April 30, 2026. LPA reviewed facility disaster plan, which was last reviewed/updated on January 31, 2026. LPA reviewed facility records for 3 staff and 3 residents. ADM stated the facility does not have any residents who take medication during day program.
No deficiencies cited during today's visit, a technical assistance was provided. This report was reviewed with Administrator Izumi Yaskawa and a copy of the signed report was provided.
