Facility Evaluation Report

On March 11, 2026, Licensing Program Analyst Manuel Monter conducted an unannounced case management – other visit to follow up regarding the change of administrator request. LPA met with Izumi Yaskawa .

During todays visit, LPA explained to ADM that the Department needs documentation showing the changes in the board. Based on the Departments records, the facility has not sent CCL documentation of the changes of the board.

LPA informed ADM that the department must confirm that the individuals that are appointing the ADM to the position, where themselves appointed by the board to their position.

LPA requested documentation showing the status of the former board members, and documentation showing the current board members being added to the board. LPA requested this documentation be sent to CCL by March 18, 2026.

No deficiencies cited. This report was reviewed with Izumi Yaskawa . A copy of the report was provided.
