Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Required 1 Year visit and met with Christina Rossi, Program Director (PD). LPA announced the purpose of the visit. LPA observed 6 residents in the living room watching television. LPA observed 7 staff including program director
During visit, LPA toured the facility inside and out. LPA toured the kitchen and dining area. LPA observed perishable food supply of at least two days and a non-perishable food supply of at least seven days. The facility has a basement where the pantry is located and has 2 refrigerators and 4 freezers. Refrigerator temperature measured with thermometer measured at 37-39 degrees F and freezer temp measured with thermometer at 0 to -2 degrees F. Residents have a separate closet for personal snacks accessible to them anytime.
LPA toured resident bedrooms. Each bedroom had available bedding and clothing storage areas as well as functioning lights. PD tested the smoke detector and found the smoke detector to function properly when tested. LPA toured 3 resident bathrooms. Each bathroom had available soap and paper towels and functioning lights. The water temperatures in the bathroom sinks measured with thermometer at 105-107 degrees F. The facility has an area for books, puzzles and board games for residents to use.
LPA toured the outside area and found the exits to be clear of obstructions. LPA observed fire extinguisher was last serviced on 05/13/2025. The facility has a Fire pull alarm that was last serviced on 12/23/25. LPA reviewed Fire and Earthquake log. The facility conducted the last fire drill on 02/03/26 and the last earthquake drill on 12/23/25
