Facility Evaluation Report

Licensing Program Analyst (LPA) Simi Rai conducted an announced case management visit to follow up on report from Department of Developmental Services (DDS) regarding the semi-annual monitoring review for the facility. LPA Rai met with Administrator, Joseph Lansana and stated the purpose of the visit. During visit, LPA Rai observed 2 staff and 3 residents at the facility.

On 11/20/2025, DDS conducted an on-site monitoring review. Two out of three staff did have current First Aid Certificate on file. The administrator stated that he would have staff (S1) enrolled in a course. There were two brown bags filled with resident's medications and they were not logged on the Centrally Stored Medication Log.

During today's visit, Administrator, Joseph Lansana stated the items stated on the semi-annual monitoring review were not discussed and staff were able to provide the necessary documents during the visit. Administrator stated there was no plan of Correction since those items were in compliance during the visit.

LPA Rai reviewed a copy of First Aid Certificate for staff (S1). LPA Rai observed the medication cabinet and did not observe a brown bag of medications. LPA Rai observed medications delivered for administration in January 2026. Administrator provided the Centrally Stored Medication log for the medications centrally stored in the medication cabinet. At random, LPA Rai reviewed resident's medications were logged on the resident's Centrally Stored Medication log.

No deficiencies were cited, as per California Code of Regulations, Title 22. Exit interview was conducted with Administrator, Joseph Lansana and a copy of this report was provided.
