Facility Evaluation Report
Licensing Program Analysts(LPA) Steve Chang conducted an unannounced annual inspection, and met with Administrator (ADM) Jemanesh Duguma. LPA observed 3 staff and 3 clients in the facility.
Facility License, ADM certificate, and Resident Personal rights were posted in the facility.
LPA toured the facility inside and out including: kitchen, living room, office area, dining area, family room, laundry room, garage, 5 resident rooms and 2 restroom. LPA observed perishable food supply of at least two days and non-perishable food supply of at least seven days. Refrigerator temperature was observed at 37 degrees F and freezer temperature was 0 Degrees F.
LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked. Room temperature was at 72 degree F, and hot water temperature was measured at 118 degrees F. LPA inspected the facility first aid kit and it was observed to be complete. The facility has emergency lighting system. The facility was equipped with smoke and carbon monoxide detectors. Smoke detectors functioned properly when tested. Fire extinguishers were last serviced on 6/19/25. The facility conducted their last fire drill on 11/7/25.
The front yard and backyard of the facility was also inspected. There was no obstruction to block the exits.
LPA reviewed facility records for 2 staff and 2 clients files.
No deficiencies were cited during today's visit. Exit interview was conducted with ADM. Report was reviewed and a copy was provided to ADM.
