Facility Evaluation Report
Licensing Program Analyst (LPA) Steve Chang conducted an unannounced case management visit and met with Administrator (ADM) Virgil Valin.
The purpose of today's visit is to check the status of the construction of the ADU of the facility.
On 4/9/2025, the Department received a notice the construction of ADU of the facility with city permit.
LPA interviewed ADM, ADM stated he/she already submitted an action plan to provide the health and safety to facility residents. ADM stated he/she will notify CCL office when the construction is completed. ADM stated construction workers have different entrance/exit from the facility.
LPA toured the backyard with ADM. LPA did not see construction workers at backyard.
Exit interview was conducted with ADM. The report was provided to ADM for review and signature. a copy of the report was provided to ADM.
