Facility Evaluation Report
Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced annual inspection and met with Administrator (ADM) Virgil Valin. ADM states the facility has 5 clients. LPA observed 5 clients, 2 staff and ADM.
LPA toured the interior and exterior of the facility with ADM to include the kitchen, client rooms, dining room, bathrooms, back and front of the facility. All exit and passageways were free and clear of obstruction.
During tour of backyard, LPA observed wooden boards and planks, with black plastic piping in a rectangular shape. ADM states the backyard is currently under construction. LPA stated to ADM, per Title 22 regulations, p rior to construction or alterations, all licensees shall notify the licensing agency of the proposed change. ADM stated the facility will submit the building permit to the Department by 4/11/2025.
LPA toured the kitchen area and observed a perishable food supply of at least two days and a non-perishable food supply of at least seven days. LPA observed refrigerator temperature at 37 degrees F and Freezer at 0 degrees F.
LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to clients in care.
