Facility Evaluation Report

Licensing Program Analyst (LPA) Simi Rai conducted an unannounced annual inspection focusing on infection control. LPA Rai met with Administrator (ADM) Kiros Woldegiorgis.

During visit, LPA Rai toured the facility to include the family room, 3 resident rooms, 2 bathrooms, laundry area, dining area, garage and exterior. All fire exit routes are free and clear of obstruction. Toxins and sharp objects were secured. LPA Rai observed 4 residents in the family room and 2 residents in their room. LPA Rai observed 4 staff members present at the facility, including the Administrator.

LPA Rai observed renovations occurring in the kitchen and second family room. ADM stated the renovations will end in 2 weeks. ADM will submit to LPA Rai a written plan of action by the end of today, February 28th 2023. The written plan of action will outline the facility's plan ensure the health and safety of the residents while a section of the facility is under construction.

Facility observed to have a designated central entry point to include a sign-in sheet and temperature check. Facility clean and disinfect twice daily and as often as needed. Bathrooms supplied with hygiene products and hand washing sign. Trash can with lid observed. LPA observed a sufficient amount of Personal Protective Equipment (PPE).

No deficiencies were cited per California Code of Regulations, Title 22. Technical Assistance Note was provided.
