The facility was cited the following Type B deficiency on July 31, 2026

87465 Incidental Medical and Dental Care Services (h)(5), POC due date, August 7, 2026

LPA received plan of corrections for type A and type B deficiency by POC due date. Deficiencies cleared during todays visit. POC cleared letter provided to ADM

No deficiency was cited during todays visit. This report was reviewed with Staff Melani Ablan. A copy of this report was provided.
