Facility Evaluation Report
On December 29, 2025 Licensing Program Analysts Manuel Monter, Steve Chang and Licensing Program Managers Romeo Manzano and Christine Dolores met with Eduardo Carranza (spouse), Jason Carranza, Ryan Carranza, Charlotte Carranza, Vanessa Carranza and Ernie Manaois in the office to follow up with their transition plan upon the death of licensee and discussed required documents.
Licensee Helen Carranza was licensed to operate 4 ARF facilities, as a sole proprietorship and owned all 4 facility properties (Rossmore ARF, Carranza #2 ARF and Valdez Care Home) and was also the administrator for the those 4 ARF facilities.
On December 29, 2025, LPM Manzano spoke to Administrative Assistant (AA), Ernie Manoias. AA stated the licensee passed away on December 21, 2025. On December 26, the Department received an email from the license's son notifying the department of the licensee's passing.
The Department discussed succession plan with Eduardo Carranza. During today's visit, Eduardo Carranza informed the Department the current plan of action is to continue operation in all 4 facilities. The Department informed Eduardo Carranza that if he would like to continue running the adult residential facilities, they would in turn need to apply for a new license for each facility.
