The inspector’s account

Licensing Program Analyst (LPA) Steve Chang Conducted an unannounced Plan of Correction visit to follow up the case management conducted on 2/5/2025, and met with House Manager (HM) Michael Vu.

The facility was cited as Title 22 section 87468.1(a)(2) Personal Rights (a)(2) Each resident shall be accorded safe, healthful and comfortable accommodations, furnishings and equipment.

On 1/24/2025, around 11:00AM, resident R1 was found missing in the facility. The facility staff called police. Around 3:00PM R1 was found by police officers 1.7 miles away from the facility. R1 was sent back to the facility around 3:45PM.

On 2/6/2025, the Department received a plan of correction..

LPA toured the facility with HM. HM tested 7 exit door alarms and all 7 door alarms were observed working. LPA observed 2 staff working at the facility.

LPA discussed with HM regarding care and supervision to residents. LPA discussed with HM regarding the updated appraisal needs and service plan. LPA discussed with HM about the reporting requirement. LPA discussed with ADM regarding the resident behavior assessment. LPA discussed with HM regarding the staff training. LPA reviewed the staff training log. LPA discussed with HM regarding to identify high risk residents. LPA discussed with HM regarding more often checking on residents. LPA obtained R1's daily behavior notes.

No citation noted today. Exit interview was conducted with HM. The report was provided to HM for review and signature. A copy of the report was provided to HM.
