Facility Evaluation Report

Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to deliver amended findings for complaint 26-AS-20250909093332, and to amend a Type A deficiency to a Type B deficiency for a Case Management (CM) visit conducted on 5/21/2025. LPA met with Executive Director (ED) Karen Nickolai. LPA stated the purpose of the visit.

During today's visit LPA delivered amended complaint findings from unsubstantiated to substantiated for complaint for 26-AS-2025090909333. A deficiency is being issued for this complaint.

LPA amended a Type A deficiency issued on 5/21/2025 to a Type B deficiency. An amended LIC809D was provided to ED during today's visit.
