Licensing Program Analyst (LPA) Steve Chang conducted unannounced annual inspection visit and met with Administrator (ADM) Lihuei Yau.
The day program is currently not in operation status. There are no clients observed on site.
LPA conducted a walk through of the facility with ADM. 3 activity rooms, kitchen, 2 bathrooms, and exterior. All fire exits routes are free and clear of obstruction.
Exit interview was conducted with ADM. The report was provided to ADM for signature. A copy of the report was provided to ADM.
