Facility Evaluation Report

Licensing Program Analyst (LPA) Christine Dolores arrived announced to conduct the facility's annual required - 1 year inspection. LPA met with Licensee, Lihuei Yau.

The day program is currently closed and not in operation. There are no clients observed on site. Licensee plans to re-open the day program in the near future with no expected date of reopen.

LPA conducted a walk through of the facility with the Licensee to include the activity rooms, kitchen, bathrooms, and exterior. All fire exits routes are free and clear of obstruction. Fire extinguisher last serviced on 09/12/2024. LPA observed first aid kits. The facility currently does not have working power and is working with PG&E to install a smart meter.

No deficiencies were cited per California Code of Regulations, Title 22.

This report was reviewed with Licensee, Lihuei Yau and a copy of the report was provided.
