Facility Evaluation Report
Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced annual inspection visit, and met with Administrator Erwin Dumantay. During the visit, LPA observed 5 residents and 3 staff. LPA explained the purpose of the visit. Homepark Care Home is a level 5 home.
LPA toured the facility inside out with ADM which included the Living room, kitchen, dining room, 3 restrooms and 4 residents bedrooms. There was no obstruction to block the walkways. The staff area of the facility was also inspected. The front yard and backyard were inspected.
While touring bedroom 1, LPA observed resident R2 laying in a bed with half side rails. LPA requested the doctors order for the half side rails for review. ADM stated he did get an order for the half side rails, but cannot find them. ADM stated he would get another copy of the order tomorrow and would send LPA a copy.
Two-day perishable food supplies and seven day nonperishable food supplies were observed. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 70 degrees F, and hot water temperature was measured at 112 degrees F in resident bathrooms.
Fire extinguisher was serviced in December 11, 2025. The facility was equipped with smoke and carbon monoxide detectors. Smoke detectors was tested by ADM, and were functional. LPA observed facility first aid kit and facility fire/earthquake drill log. ADM stated he conducted a fire drill on December 2, 2025, but stated the drill log was at his home office and not in the facility to review. Page 1 Out of 2.
