Facility Evaluation Report
On 04/22/25 Licensing Program Analyst Marcela Yanez conducted an unannounced Case Management visit and met with Administrator, Audrey Salvador. LPA announced the purpose of the visit. LPA observed 1 staff including Administrator.
The purpose of todays case management visit is regarding Licensee not notifying the department of Administrator change within 30 days of November 15, 2023 when Julius Ervin Javier resigned as Administrator. LPA Yanez reviewed records submitted and verified the Department was not notified of Administrator change by the Licensee. ADM Audrey Salvador provided documentation of email sent to the department on February 21, 2025 notifying the the department to change Administrator from Julius Ervin Javier to Audrey Salvador.
During visit LPA Yanez obtained copies of documents submitted to the department on Feb 21, 2025. The current ADM is still showing Julius Ervin Javier.
A Deficiency is cited during todays visit, see Case Management dated 04/22/2025 LIC809-D. This Report was reviewed with Administrator, Audrey Salvador. A copy of the report was provided and appeals rights.
