Facility Evaluation Report
On 05/09/25 Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced POC visit. LPA announced the purpose of the visit. LPA met with Administrator (ADM) Audrey Salvador. LPA observed 4 residents and 2 staff.
on 04/29/25 the facility was cited for type B deficiency for not reporting to the department of the new administrator within 30 days.
(b) The licensee shall notify the licensing agency, in writing, within 30 days of a change of administrator.
on 05/08/25 ADM attended the required training for HIV/TB training and provided a copy of the Webinar HIV/TB training evaluation for attending the webinar on 05/08/25.
On 05/09/25, LPA conducted a POC visit to clear the Type B POC. A copy of the Letter of Deficiencies Cleared letter was provided.
No deficiencies cited during today's visit. This report was reviewed with Administrator Audrey Salvador and a copy of the signed report was provided.
