On 6/17/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct the Annual 1-year required inspection. LPA Calandra knocked on the door three times and rang the doorbell but no one came to the door. LPA Calandra observed a silver vehicle in the driveway and blinds in the front closed. LPA Calandra called the Administrator/Licensee and left a voicemail requesting a callback.
LPA to return at a later date to complete the Annual inspection. No signature obtained on report as Administrator/Licensee was unavailable for the visit.
