The inspector’s account

On, 2/23/2026, LPA Grace Donato, conducted an unannounced annual inspection visit. LPA met with Administrator Janessa Flores and LPA explained the purpose of the visit

LPA toured the facility inside out which included the Living room, kitchen, dining room, 2 restrooms and 4 resident bedrooms. The front yard and backyard were inspected. There was no obstruction to block the walkways.Th e facility is observed to be clean, odorless, and well maintained. Residents bedrooms were observed to be well organized and fully furnished with adequate lighting. Rooms have code locks.

Two-day perishable food supplies and seven day nonperishable food supplies were observed. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 70 degrees F, and hot water temperature was measured at 107 degrees F.

The facility was equipped with smoke and carbon monoxide detectors. LPA observed facility first aid kit and facility fire/earthquake drill log. The facility's last drill was on January 19, 2026. LPA reviewed facility records for 4 staff and 5 residents. LPA also reviewed resident medications and centrally stored medication records.

No deficiencies cited today. Report is reviewed and copy is provided.
