Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management visit and met with Administrator/CEO Maria Nicolacoudis.
The purpose of the visit was to follow up on an incident reported by the facility via LIC624 Unusual Incident/Injury Report (IR) on 10/30/2025. The IR stated that on 10/29/2025, at approximately 4:15 PM, facility staff noticed that resident R1 could not be found. R1 eloped from the facility through a rear gate and was later found by facility staff at a nearby park at 5:15 PM.
During visit, LPA Marrufo interviewed R1 and staff S1-S3. LPA toured the facility and walked the path that staff stated R1 took to elope from the facility.
During visit, LPA Marrufo obtained copies of R1's Physician's Report, Staff Schedule for 10/29/2025, invoices for new alarms to be installed, and 30-minute attendance log.
During visit, Administrator/CEO Maria Nicolacoudis stated that locks and alarms are being installed on the doors and gates that R1 used to elope from the facility.
LPA Marrufo requests that the facility submit an updated emergency exit plan by 11/13/2025.
No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Administrator/CEO Maria Nicolacoudis and a copy of this report was provided.
