Facility Evaluation Report
Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Cristeta Carasi Assistant Program Director (APD) and Jamie Rivera-Vallestero Program Director (PD).
LPA toured the facility with APD. Conference rooms, 4 activity rooms, recreation room, kitchen, 3 restrooms and offices were inspected. Lockers for each client were observed. Outdoor activity courtyard was observed. Dishwashing solution bottle was observed on the sink in the kitchen. APD locked it immediately in the cabinet under the sink.
5 clients and 3 staff went to outing. 26 clients and 7 staff were observed in the facility. 35 client files and 5 staff files were reviewed.
Cleaning products were observed locked. Room temperature was at 70 degree F. Fire extinguisher was serviced on 07/2/2025. The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Carbon monoxide detectors were tested by staff, and they were working. The facility has emergency lighting system. First aid box and flash light were observed in the facility.
The last time the facility conducted fire drill and earthquake drill were on 8/12/2025.
No citation were noted today. Exit interview was conducted with PD. A copy of this report was provided to PD.
