The inspector’s account

Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced annual inspection visit, and met with Staff Akhom Kham, referred to as S1. During the visit, S1 stated there are 36 clients and 11 staff in the facility. LPA explained the purpose of the visit.

LPA toured the facility inside out with S1 which included the Living room, kitchen, activity rooms, and restrooms. There was no obstruction to block the walkways. The staff area of the facility was also inspected. The front yard and back of the facility was also inspected.

Two-day perishable food supplies and seven day nonperishable food supplies were observed. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 72 degrees F, and hot water temperature was measured at 114 degrees F in client restrooms.

Fire extinguisher was serviced in April 13, 2026. The facility was equipped with smoke and carbon monoxide detectors. LPA observed facility first aid kit and facility fire/earthquake drill log. The facility's last drill was on May 12, 2025.

LPA reviewed facility records for 4 staff and 4 residents. LPA noted that Clients (C1-C3), do not have needs and services plan. Clients C1-C3's Individual program Plan is older than 1 year. S1 stated the facility does not have needs and services plans for clients. S1 stated the day program does not have any clients who need assistance with medication administration while they attend day program.
