Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit and met with assistant manager Evan Fernandez (S1) and program manager Mina Karim (MK).

LPA toured the facility with S1. Licensee and personal right posters were observed in the facility. LPA inspected the reception area, art studio, computer lab room, restrooms, office, activity room, break room, kitchen, and music/game room.

Room temperature was observed at 69 degree F. Knives and dish soap bottle were observed locked.

There are lockers for the participants and a refrigerator for the participants to store their belongings and lunches.

Carbon monoxide detector was observed not working. Staff bought a new one and installed before LPA finished the visit. Fire extinguisher was observed expires today. Staff bought a new one and installed before LPA finished the visit. First aid boxes were observed in the facility. The facility is equipped with emergency light system. The last time the facility conducted the fire drill was is 5/13/2025.

39 clients and 16 staff were observed in the facility. 5 client files and 5 staff files were reviewed.

An exit interview was conducted with Program Manager Mina Karim. A copy of the report was provided.
