Facility Evaluation Report

Licensing Program Analyst Manuel Monter and Licensing Program Manager Romeo Manzano conducted an unannounced case management visit.

On October 11, 2024, the Department conducted an unannounced case management visit. LPA and LPM meet with Akhom Kham, Day Program Assistant Coordinator(referred as S2), Mina Karim, Senior Program Manager (referred as S1) and Danielle Sanchez, Director of Operations (Referred as S3)

The purpose of this visit was discussed with with S1, S2 and S3. The Department of Social Services Community Care Licensing Division has issued a Decision and order on October 9, 2024, for staff S3. S3's Administrator Certificate for Adult residential facilities has been revoked or deemed forfeited. S3 is prohibited from preforming his/her duties and responsibility of an Administrator in an Adult Residential Facilities/ Adult Day Program. A copy of the Decision and Order was provided and discussed with S1 and S3. S3 stated he/she does not have a valid Adult Residential Facility Certificate (expired in September 2024). S3's fingerprint clearance is on pending status, which prohibits S3 to be present and work at the day program until cleared.

S1 stated that he/she has been appointed by the licensee (Corporate) as the Senior Program Manager since 2023. S1 was informed that the Department did not receive Board of Resolutions to appoint him/her as the new administrator of the facility. LPA discussed the duties and responsibilities of a day program Administrator wherein he/she is responsible to uphold the duties and responsibilities of an Administrator as per Title 22, 82064 - Administrator - Qualifications and Duties. LPA informed S1 to ensure that the Director of Operations (S3), position does not overlap with S1's position or Day Program Assistant Coordinator or in similar Capacity. S1 and S3 agreed and understood.
