Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Required 1 Year visit and met with Director Vivian Chung.
During visit, LPA Marrufo toured the facility inside and out. LPA toured the kitchen area. The food items in the kitchen were all sealed. LPA tested the carbon monoxide detector in the kitchen and observed it to function properly when tested. LPA reviewed the Fire Alarm Annual Test Certificate and observed the fire alarm system last passed its inspection on 04/17/2025. The Emergency Disaster Drill Log indicates the last drill was conducted on 11/24/2025.
LPA toured two out of two resident bathrooms. Each bathroom had available soap and paper towels and working lights. The water temperatures in the bathroom sinks ranged from 110 F to 112 F.
LPA toured the activity rooms of the facility. LPA observed clients singing and doing activities in the rooms.
LPA toured the outside area and found it to be clear of obstructions.
LPA reviewed 7 resident records and 7 staff records during visit.
