Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management visit. LPA announced the purpose of the visit. LPA met with Lead Staff Rhodelito Perez.

During an initial complaint investigation visit LPA was greeted by Staff S1 who stated he/she has been working at the facility since 05/15/26. ADM stated S1s LIC9182 was submitted to licensing on 05/11/26. The department was not able to associate or transfer to the facility because S1 has not been fingerprinted since 2007 and does have a fingerprint clearance.

ADM was informed that S1 can not be at the facility till he/she is fingerprint cleared and associated. LPA informed ADM via phone of deficiency and civil penalty of $500 dollars. See LIC809D and LIC421BG.

This report was reviewed with Designated Administrator, Rodelito Perez, ADM was notified via phonet, and a copy of the report and appeals rights were provided.
