Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management POC visit. to clear deficiencies cited on 12/17/25. LPA announced the purpose of the visit. LPA met with Lead staff Rodelito Perez.

During visit LPA toured the kitchen and observed new cabinets with new counter tops photographs taken with state provided phone. LPA tested the bathroom faucet water with thermometer and measured at 113.5 degrees F. LPA observed the bathroom screen was replaced. LPA reviewed 2 staff files and 2 resident files.

LPA received plan of corrections for type A deficiency by POC due date. Deficiencies cleared during todays visit. POC cleared letter provided to Rodelito Perez, lead staff.

No deficiency was cited during todays visit. This report was reviewed with Rodelito Perez. A copy of this report was provided.
