The inspector’s account

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Administrator (ADM) Abella Cabanayan. 3 staff and 3 residents were observed in the facility. LPA reviewed 2 resident files and 2 staff files.

LPA toured the facility inside and out with ADM. License, personal rights posters and Administrator Certificate were observed in the facility.

Living room, kitchen, dining area, 2 restrooms, 5 resident rooms, laundry room and garage were inspected. Two day perishable food supplies and seven day nonperishable food supplies were observed sufficient. The temperature of the freezer was observed at 0 degree F, and the temperature of the refrigerator was observed at 37 degree F. Medication closet, knives closet were observed locked. Dish washing soap bottle was observed on the sink of the kitchen, staff locked the dish washing soap in the cabinet under the sink immediately. Room temperature was at 68 degree F, and hot water temperature was at 105 degree F in facility. Fire extinguisher was serviced on 8/19/2025. The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Carbon monoxide detectors was tested, and were working. First aid box and emergency light system were observed in the facility.

Front yard and backyard were inspected. There was no obstruction to block the walkways. One storage room was observed in the backyard. The facility had fire drill on on 11/13/25.

No citation noted today. Exit interview was conducted with ADM. This report was provided to ADM for signature. A copy of the report was provided to ADM.
