Facility Evaluation Report

Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced Required 1-year Annual inspection. LPA was met by licensee Faraviz Foshati. Administrator (ADM) Ebadat Hassan was not at the facility. ADM was not available due to prior commitment.

LPA toured the facility inside and outside. LPA observed that the facility has no clients and 3 bedroom are currently occupied by the Licensee's family. Licensee stated that they have not been operating since the day they were licensed on 03/07/2025. Licensee stated that they were in the process of getting vendorized by the San Andreas Regional Center (SARC) and have not received the approval as of this date. LPA observed 3 bedroom, and 2 bathroom, a kitchen, a living area and a dining area. There are no files or records to review.

LPA advised licensee to notify CCLD of any changes with their license or once the vendorization has been approved by SARC.

No deficiencies were cited during today's visit based on California Code of Regulations (CCR) Title 22. An exit interview was conducted with licensee Faraviz Foshati and a copy of the report was provided.
