Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced Required 1 Year visit and met with administrator Joelito Pongyan and stated the purpose of the visit.
The facility serveis 18 to 59 years old who are ambulatory . No resident was observed at the time of LPAs visit. Staff stated that 3 Out of 4 residents were with staff who visited 1 Out of 4 resident who was taken to the hospital. LPA Partoza inspected the facility inside and outside and observed the exits were clear of obstruction. LPA Partoza i observed 4 resident rooms, 1 staff room and 2 living area, kitchen and garage.
LPA Partoza observed food supply of 2 days of perishable foods and 7 days of non-perishable foods. LPA Partoza observed locked cabinets for chemicals, medications and sharp objects.
LPA observed the washer and dryer in the garage area was in good working, chemicals are separated from the food pantry. LPA observed that 1 Out of 4 bedroom closet sliding door was not working properly and needs repair. 2 Out 2 bathroom used by residents have grab bars and anti-skid floor. The facility has a dining area and 2 living area that are free from debris and organized. The facility has 3 staff at the time of the LPAs visit.
LPA Partoza measured the hot water temperature for 2 out of 2 bathrooms and kitchen. The water temperature was measured with a digital thermometer at 140.4 to 145.9 degrees F. The facility room temperature is at 70 degree F. LPA observed two refrigerator, freezer combination, 1 in the garage used by staff and overflow of perishable food items, and the other one is located in the kitchen. The refrigerator temperature measured at 32 degree F and freezer is at 0 degree F.
