Facility Evaluation Report
On 9/3/2025, Licensing Program Analyst (LPA) Maria (Mita) conducted an unannounced case management visit - regarding an incident that was reported to CCLD by the Licensee. LPA met with administrator Joelito Pongyan and stated the purpose of the visit.
The facility serves 18 to 59 years who are developmentally disabled.
On 09/03/2025 at 8:43 a.m., the Department received a call from the Licensee stating a resident (R1) was sexually abused by a staff (S1).Licensee stated that on 09/01/25, S1 had an inappropriate physical contact with R1. Licensee stated that he/she was made aware of the incident at approximately 5:00 p.m. of 09/02/25.
On 09/03/2025 at 3:35 p.m. LPA conducted the initial 10-day visit and requested for the following documents; Copies of 4 out of 4 resident's (R1 to R4) records such as but not limited to LIC 602, IPP (Individual Program Plan), progress notes, and emergency contact information. LPA requested for copies of 4 out of 4 staff (S1 to S4) records such as but not limited to training and LIC 501 and staff schedule. Based on review of the staff record, 6 staff present during the visit were fingerprinted and background cleared.
LPA determined that the case will require further investigation and will remain open until further notice.
No deficiencies were cited during today's visit based on California Code of Regulations (CCR) Title 22. An exit interview was conducted with administrator Joelito Pongyan and a copy of the report was provided.
