Facility Evaluation Report
Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced Required 1-year Annual inspection. LPA was met by licensee Merlita Patio. Ferdinand Patio was not available due to prior commitment.
LPA toured the facility inside and outside. LPA observed that the facility has no clients and 3 bedroom were under construction. Licensee stated that they have not been operating since the day they were licensed on 12/05/2011. Licensee stated that they were not able to complete the required program design required by the San Andreas Regional Center (SARC) for the level of care they were applying for (level 4F). They have decided to retain the CCLD license, but not receive any clients until they are able to meet the SARC requirement.
LPA observed 5 bedroom, and 2 bathroom, a kitchen, 2 living area and a dining area. The 3 bedroom for future residents are not in a livable condition. 2 of the room are used by the licensee and his/her family.
There are no files or records to review. Licensee stated that the facility is not ready to be inspected by the Fire Marshall. On 12/26/2024, LPA Steve Chang, have inspected and requested for the facility floor plan.
No deficiencies were cited during today's visit based on California Code of Regulations (CCR) Title 22. An exit interview was conducted with licensee Merlita Patio and a copy of the report was provided.
