Facility Evaluation Report

Licensing Program Analysts (LPA) Marcela Yanez conducted a facility closure visit and met with Administrator/Licensee, ADM/LIC, Thu Nguyen

On 06/27/25 ADM/LIC submitted a letter to the Department announcing the facility will be closing.

During visit, LPA toured the facility and observed that there were no residents present. LPA observed resident bedroom as empty, with no resident belongings. ADM/LIC confirmed that all residents had been moved out of the facility as of June 10, 2022 since facility was in non operational. ADM/LIC provided original license to LPA

No deficiencies were cited at this time as per California Code of Regulations Title 22.

This report was reviewed with Administrator/Licensee Thu Nguyen and a copy of this report was provided. LPA informed ADM/LIC that she could complete the closure survey once received.
