Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Required 1 Year visit and met with Thu Nguyen, Administrator/Licensee (ADM/LIC). LPA announced the purpose of the visit. LPA did not observe any residents or working staff at the facility
This facility status is non operational, ADM stated this facility is no longer overseen (vendorized) by San Andreas Regional Center (SARC) as of 03/07/25 ADM/LIC provided letter. ADM/LIC was informed that the facility is still licensed by CCLD and is able to accept residents under RCFE license. ADM/LIC stated that she is unsure if she will continue to operate as a RCFE.
LPA observed that the facility still has working electricity, and water. The kitchen appliances were still in working order during the visit.
ADM/LIC is aware the annual fee is still due and the facility can not be rented out because it is still a licensed facility. ADM/LIC was informed that the current licensing fees of $495.00 are due on 06/27/25 and if not paid will accrue late fees and penalties.
ADM was informed that the facility is in active status, and they are not exempt and still need to adhere to Title 22, Division 6 Policy, Regulations, and Laws on RCFEs.
LPA advised ADM/LIC if she accepts a new resident to inform the department.
No deficiencies were cited at this time as per California Code of Regulations, Title 22.
