Facility Evaluation Report
On April 24, 2025, LPA Monter conducted a case management POC visit. LPA met with Lead staff, Nelson Reguindin.
On February 25, 2025, the Department received a complaint alleging the facility is not meeting the hygiene needs of residents in care. It has been alleged that resident R1 was observed with poor dental hygiene and wearing 2 diapers.
On April 16, 2025, LPA Monter concluded a complaint investigation. The following deficiencies were cited during the visit with a POC due date of April 17, 2025 and were cleared during today’s visit.
