The inspector’s account

Licensing Program Analyst (LPA) Manuel Monter arrived unannounced to conduct a case management – legal/non-compliance visit. LPA met with Licensee Evelyn Canonizado. LPA observed two staff members and 3 residents.

The purpose of the visit to ensure the facility is adhering to the compliance plan submitted to Community Care Licensing (CCL) after a non-compliance meeting held on September 19, 2025

During visit, LPA toured the facility with ADM to include the resident bedrooms, bathrooms, common areas. There was no obstruction to block the walkways. The staff area of the facility was also inspected. The front yard and backyard were inspected. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care.

LPA reviewed facility Care & Supervision protocol for known or identified behaviors, such as but not limited to elopement, wandering, aggression, property destruction, and confusion. This protocol addresses levels of supervision, prevention & response procedures, documentation & Communication, and staff training,

Based on the non-compliance plan, training was to be completed by a certain date to address resident behaviors such as but not limited to; elopement, wandering, aggression, property destruction, and confusion. The training document contains the topic, date, name and signatures of the participants.
