The inspector’s account

On 03/27/2026 Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced Case Management visit to conduct a Non-Compliance Plan (NCC) Quarterly Visit and met with Lead Staff (LS) Tessie Hernandez. LPA announced the purpose of the visit. LPA observed 1 resident watching tv and 2 in their rooms. 1 out of 4 residents at day program. LPA observed 1 staff. LPA called Administrator to let her know of the NCC visit.

The purpose of the visit is to ensure the facility is adhering to the Compliance Plan submitted to Community Care Licensing (CCL) after an informal meeting held on 09/15/25.

During Visit LPA took a photo of schedule for week ending 2/3/26-03/1/26 and week ending 3/29/26 which showed administrator at the facility Monday thru Friday from 8AM to 12 PM. Staff stated ADM was not at the facility on 03/27/26, ADM stated she had an emergency and not able to come to facility. LPA reviewed 2 residents file. LPA toured the facility and observed resident rooms. LPA observed bedding, lighting. LPA tested the water temperature in bathroom water temperature measured with thermometer at 96.5 degrees F.

LPA toured the facility and observed the facility gate leading to the front yard was locked with a metal lock (Photo taken with state provided phone). LS removed the lock and stated R1 likes to go and rummage thru garbage.

Deficiency was cited as per California Code of Regulations Title 22. This report was reviewed with Lead Staff Tessie Hernandez and a copy of this report and appeal rights were provided.
