Facility Evaluation Report

On July 24, 2025, Licensing Program Analyst Manuel Monter conducted an unannounced Case-Management-other regarding the facility being interested as being potentially used as an emergency shelter during an emergency. LPA met with Assistant Program Director (AD) Tiana Musika. LPA explained the purpose of the visit.

During the visit, LPA and AD toured the facility inside and out, which included: 9 classrooms, kitchen, office areas, and restrooms. The staff area of the facility was also inspected. Front yard and backyard of the facility was also inspected. LPA observed various clients doing activities during visit.

LPA interviewed AD regarding the facility being interested as being potentially used as an emergency shelter during an emergency.

No deficiencies cited during today's visit. This report was reviewed with Staff Assistant Program Director Tiana Musika and a copy of the signed report was provided.
