The inspector’s account

On July 14, 2025, Licensing Program Analyst Manuel Monter conducted an unannounced Case-Management incident visit. LPA met with Assistant Program Director Tiana Musika . LPA explained the purpose of the visit.

On July 1, 2025, the facility was cited the following type A deficiencies during a case management -Incident visit. The following code sections were cited on July 1, 2025, with a POC due date of July 2, 2025:

1. 82078 Responsibility for Providing Care and Supervision (a)

On July 14, 2025, LPA conducted a POC visit to clear the Type A POC's. LPA received a copy of staff training conducted regarding: importance of safety of individuals if ID/DD.

A copy of the Letter of Deficiencies Cleared letter was provided.

On July 11, 2025, the Department received an incident report for resident R1. The incident report stated, on July 8, 2025, staff observed an area in R1's privates appeared red and swollen while being changed. Staff noted R1's reaction and physical symptoms were unusual compared to previous diaper changes.

Based on a review of R1's Individual Program Plan, R1 resides with his/her family.
