Facility Evaluation Report

Licensing Program Analyst (LPA) conducted a facility closure visit and met with Administrator Elizabeth Reyes (ADM).

LPA conducted a walk-through of the facility and inspected all rooms and common areas. LPA observed 2 residents (R1, R2), and 2 staff (S1, S2). LPA observed S1 and S2 were packing stuff. ADM, S1 and S2 still provide care and supervision today.

2 residents already relocated to other facilities. 2 residents (R1 and R2) were observed in the facility resident room. R1 and R2 stated they want to stay in the facility. They understand there will be a new management team takes over the facility 7/15/2025.

ADM stated another resident R3 went to day program and will come back by 3:00PM. ADM stated residents (R1 - R3) will stay in the facility under the new management.

A Supported Living Services, Individual Choices Inc. with San Andreas Regional Center’s consent and approval, will retain the 3 clients (R1, R2, R3) under Support Living Services Program staring 7/15/2025. Individual Choices will support residents to live independently, to make meaningful choices, build relationships, participate in their community while maintaining a safe and stable life.

LPA did not observe other residents in the facility. ADM surrendered the original facility license to the LPA.

Exit interview was conducted with ADM. The report was provided to ADM for review and for signature. A copy of the report was provided to ADM.
